By completing and submitting the application form on the www.flipflopgym.co.za website I have acknowledged
to reading and understanding the following;
I give consent for my child to take part in pre and primary school gymnastics and events - including games and use of school
equipment and props. I understand and accept that all activities shall be undertaken at my child’s own risk and
that reasonable precautions will be taken for the safety and welfare of my child. I hereby indemnify Flipflop
Gymnastics together with all employees, staff members, contractors, agents, representatives and the like in their
personal capacity, against any liability for any loss, damage, death, illness of whatsoever nature (whether direct,
consequential or otherwise) and against all or any claims for personal or other injuries that may be sustained from my child’s
participation in all activities, as well as any medical fees and expenses and including any expenses relating to
physio therapy treatments. Flipflop Gymnastics together with all employees, staff members, contractors, agents, and
representatives shall not be liable for any negligence on their part.
The below clauses (a and b) only apply to parents paying Flipflop Gymnastics directly.
I hereby consent that, and authorise Flipflop Gymnastics or agent to, at all times: -
a) contact, request and obtain information from any previous consumer or registered credit bureau relevant
to seeking references and consumer history and to assist in conducting a consumer assessment in respect of
myself;
b) to furnish personal consumer information concerning myself to any credit bureau, or to any prospective
consumer seeking references and consumer history and to assist in conducting a consumer assessment in
respect of myself.
Each provision herein is severable and if any provision herein is found to be invalid, unenforceable or unlawful in
any jurisdiction, it shall, to the extent that it is so invalid, unenforceable or unlawful, be struck from the
document as if it were not included, without invalidating or affecting the remaining provisions of this document,
which shall remain of full force and effect.
